How Outreach works
You can access the service through by speaking to one of the Outreach team. They will discuss your enquiry with you and, if appropriate, arrange for a visit to your home to discuss possible ideas or solutions.
Our team of volunteers bring a quality to caring that is different from care provided by health care professionals. They are specially trained and recruited for their work within patients' homes and have undergone the standard checks required when working at Great Oaks.
What is Outreach?
Great Oaks Hospice provide Outreach services. The service aims to support and improve the lives of people with cancer (or any other life limiting diagnosis) and their families, within their own homes. It takes the services provided at Great Oaks into the community, whilst linking in with other local service providers. Note that it is a ‘non-nursing’ service.
